Recent projects:

Interoperability

We are currently working with the Hampshire and Isle of Wight End of Life Care (EoLC) Interoperability project, commissioned by Solent NHS Trust, documenting requirements for sharing information between different health and social care systems in the region, with the aim of providing joined-up urgent and emergency EoLC.

We have over 40 years’ combined experience working in palliative care and EoLC and we aim to assist the project team in finding and implementing an EoLC Interoperability Solution.

Hospices

We have worked together with over 12 different hospices across the UK designing, developing, implementing, re-developing and supporting their electronic health care systems.

We also have extensive experience with hospice reporting at a national, local, service and individual patient level, having worked on some of the recent national reporting projects and having implemented and reported on Outcome Assessment and Complexity Collaborative (OACC) Outcome Measures in a number of different hospices.

Care Homes

We are leading on a project, commissioned by the Isle of Wight Clinical Commissioning Group (CCG), to implement SystmOne into care homes, in order to provide the best possible care for residents, by sharing information between healthcare professionals and professional carers. We have recently gone live with our first Care Home, in Ryde, and will shortly be going-live with a second home.

We have a blueprint, care homes SystmOne module that can be implemented quickly and in a cost-effective manner.
